Healthcare programs

The implementation of the program is temporarily suspended for the period of martial law in Ukraine. 

Categories of citizens who can receive assistance from the Foundation: 
  • People from large families 
  • People with disabilities of groups 1 and 2 
  • Pensioners 
  • Veterans and people with the status of “Children of the Great Patriotic War” 
  • Disabled children from low-income single-parent families 
  • Single mother/father raising a child under the age of 18 
  • Large families (with three and more children under the age of 18) 
  • Families raising orphaned children, children deprived of parental care, and children whose parents were deprived of parental rights 
  • Orphans under the age of 23 
  • Families with more than one seriously ill family member. 
The implementation of the program is temporarily suspended for the period of martial law in Ukraine. 
 

1. An application from the first person (for minors or adults recognized as incapacitated, the application shall be written by an official representative – parents/guardian). The application shall be addressed to the Director of the Foundation (ICF in Honour of the Intercession of the Most Holy Mother of God) Dmytro Sergeyev in an arbitrary form and shall be certified by the applicant’s signature. It shall have a date when it was written and the contact phone number. 
 
2. Copies of the applicant’s passport and identification code (RNOKPP). 
 
3. Copy of the patient’s birth certificate/passport and identification code (if the applicant is an official representative/guardian of the person in need of aid). 
 
4. Copy of the patient’s disability certificate/pension certificate, including the page where the validity period is indicated. 
 
5. Medical report/patient discharge summary (epicrisis) with full diagnosis and recommendations for further treatment/examination, including the signature and seal of the doctor, and the stamp of the healthcare institution issuing the document. 
 
6. !For the guardian – a copy of the document confirming the guardianship. 
 
7. Consent to the processing of personal data and posting of photo/video materials on the Foundation’s website and on the Foundation’s social media accounts. 
 
8. Invoice from the clinic/laboratory for payment of medical services. 
 
9. Certificate of income from the place of work + certificate of receipt of social aid/pension (if any) for six months. 
 
All copies of the documents must be certified copies. Each copy of the documents shall have a hand-written note on it: “True copy”, as well as the date, and signature of the applicant.

After you have completed medical treatment/examination, you need to provide the Foundation with a copy of the relevant report/discharge summary. 
 
Mandatory requirements for documents: 
  • Copies should be taken from the original documents! Copies should be of good quality, all the details of the document should be clearly visible. 
  • Copies should be taken in a solid background and displayed fully without cutting the edges of the document. All four corners should be displayed and copies should not contain unnecessary objects and inscriptions that are not on the original document (for example, paper clips, pencils, fingers, edges of document covers, etc.). 
  • Copies of documents not taken from the original documents, edited in a graphic editor, poor-quality photographs of documents will not be accepted for consideration.

An application from the official representative of the child (parents/guardian) with a request to help cover the cost of the rehabilitation course (the application shall be addressed to the Director of the ICF in Honour of the Intercession of the Most Holy Mother of God Dmytro Sergeyev. It is written in an arbitrary form and is certified by the applicant’s signature. The application shall have the date when it was written and the contact phone number). 
 
2. Copies of the applicant’s passport and identification code (RNOKPP). 
 
3. Copy of the patient’s birth certificate/passport and identification code (RNOKPP card). 
 
4. Copy of the patient’s certificate of disability (pages 1 and 2 are mandatory!), including the page where the validity period is indicated. 
 
5. Advisory opinion of the family doctor/paediatrician (medical certificate) on the absence of medical contraindications to the rehabilitation course. 
 
6. !For the guardian – a copy of the document confirming the guardianship. 
 
7. Consent to the processing of personal data and posting of photo/video materials on the Foundation’s website and on the Foundation’s social media accounts. 
 
8. Invoice for payment of the cost of the rehabilitation course.

Documents for payment of the rehabilitation course are accepted no earlier than two months before the start of the course. Submission of documents to the Foundation DOES NOT mean the guaranteed payment for medical treatment. The decision to pay or reject in payment for treatment is made one month before the start of the rehabilitation course. After that you will be contacted by a manager of the project Rehabilitation of Cerebral Palsy to confirm that rehabilitation is still relevant and clarify further details for its payment. 
 
If the patient has not completed the rehabilitation course (failed to attend the course for any reason, did not complete the rehabilitation course, etc.), it is mandatory to inform the Foundation’s representative about this. Otherwise, the patient will be excluded from the project. 
 
After completing the rehabilitation course, the patient shall provide the Foundation with a copy of the statement on the completion of the rehabilitation course. 
 
Documents, including the statement of the completion of rehabilitation, sent electronically (scanned/photo copies sent to e-mail, Viber, WhatsApp, Telegram, etc.) will not be considered.

Help to the UOC

Archpriests of parishes and monasteries of the Ukrainian Orthodox Church 
1. An application from the religious community certified by resolution of the Ruling Diocesan Archiereus (bishop) – with a request to provide aid. The application shall be addressed to the Director of the ICF in Honour of the Intercession of the Most Holy Mother of God Dmytro Sergeyev. The application shall be made on the official letterhead of the parish, including all contact details and bank details. (The request shall be written in an arbitrary form. It shall be certified by the signature of the head of organization and the stamp of organization. An outgoing reference number and date shall be indicated in it, as well as the phone number of the contact person for further cooperation on this project). 
 
2. Copies of the registration documents of the church/community certified by the signature of the head or an authorized person and the stamp of the organization: 
  • Extract from the Unified State Register of Enterprises and Organizations of Ukraine (EDRPOU) 
  • Decision on non-profitability 
  • Articles of Association: the first three pages and the last page 
  • Order on appointment as archpriest (church priest) 
  • Certificate of the priest (for church priest) or a document confirming the authority (for the authorized person) 
3. Statement with bank details of the religious community (+IBAN). 
 
For renovation and construction works, the following shall be provided additionally:

Copies of documents for the land plot where construction and renovation works will be performed or are being performed, as well as copies of documents for the premises where construction and repair works will be performed or are being performed (to be certified by the signature of the head of organization or an authorized person, and the stamp of the organization). 
 
5. A clear description of the construction work plan indicating the cost and term of work performance (cost estimate – additionally) with the signature of the head of organization and the stamp of organization.

  • Copies of documents not taken from the original documents, edited in a graphic editor, poor-quality photographs of documents are not accepted for consideration.

Humanitarian Aid

ATTENTION! Applications are accepted solely through the electronic form of the request for assistance and in accordance with the said procedure for submission/processing of appeals.

Humanitarian aid in the form of food is provided to citizens who are in the most critical position, namely:
  • Families with children, pensioners, persons with disabilities 1- 2 groups, and citizens who care for incapacitated persons in Donetsk, Lugansk, Zaporizhia, Kharkiv, Mykolaiv, Kherson, Dnipropetrovsk regions. (Proofing documents: birth certificate+passport of one of the parents / certificate of disability / pension certificate. Delivery address must be in line with the specified region).
  • Families who have lost a breadwinner as a result of a full -scale invasion. (Proofing documents: death certificate and family relations document).
Important! Applications are accepted exclusively through the electronic aid request form, and in accordance with the specified procedure for filing/processing the request.

Humanitarian aid in the form of a grocery set is provided to citizens who are in the most important situation, namely:
  • Families with children, pensioners, persons with disabilities 1- 2 groups, and citizens who care for incapacitated persons in Donetsk, Lugansk, Zaporizhia, Kharkiv, Mykolaiv, Kherson, Dnipropetrovsk regions. (Proofing documents: birth certificate+passport of one of the parents / certificate of disability / pension certificate. Delivery address must be in line with the specified region).
  • Families who have lost a breadwinner as a result of a full -scale invasion. (Proofing documents: death certificate and family relations document).
If you belong to one of the above categories, you must fill out the form of assistance, which is at the end of this page, as well as at the "Contacts" page.

Applications/Documents through other communication channels (Facebook, telephone, messengers) are not accepted.

Requirements for filling out in the form:
  • Carefully indicate the exact delivery address in accordance with the fields of the form. According to the data provided, a delivery invoice will be formed, so the applicant is responsible for the correctness of the information provided.
  • Attach a document that confirms belonging to the category of aid recipients determined by the Foundation. Applications without documents are not accepted. !!! If you cannot attach the document to the form, most likely that its size exceeds the maximum permissible, 2.5 MB. You need to reduce the size and send the form with the documents.
After processing your application, you will receive SMS notification from the postal operator with delivery information. The processing speed will depend on the number of queries received.

Upon receipt of the parcel in the post office, make sure that the package is in good condition, and the weight of the parcel is at least 9.6 kg. If one of the criteria is not true, please refuse to obtain a parcel and draw up an act # 51.

We draw your attention to the fact that applications that do not belong to the categories of aid recipients or do not contain relevant confirming documents are not accepted for consideration.

The application shall be made on the official letterhead of the organization, containing all contact details and bank details. The request shall be written in an arbitrary form. It shall be certified with the signature of senior executive and the stamp of the organization (or an authorized person). An outgoing reference number and date shall be indicated in it, as well as the phone number of the contact person for further cooperation on this project. 
 
2. Copies of registration documents of the church/community certified by the signature of the head of organization or an authorized person and the stamp of organization:
